The Prophet 5/10 rev4 doesn't work with MRCC via USB. I reported that months ago. It should be deleted from the list of compatible devices. (No problems via 5-pin MIDI.)

Yes, the MIDI port names are provided by the device. Unfortunately renaming is not possible in every DAW. Live for instance doesn't offer renaming of MIDI ports.
